IMMUNIZATION REQUIREMENTS

Immunizations

Indiana State Law requires that all students show proof of basic immunizations. Please see your family doctor before departing your home and bring copies of your immunization records with you. If you do not have proper records, the school nurse will assist you in receiving the immunizations once you have arrived on campus.
Please note, if you wait and get your immunizations here, it might be expensive (approximately $350.00). You must complete all required immunizations in order to continue with class attendance.

  • Tetenus and Diphtheria toxoid or Tdap - within last 10 years
  • Meningitis vaccination
    If you do not wish to receive the meningitis vaccination, you can read about meningococcal meningitis and sign the Meningococcal Risk Acknowledgement Waiver
  • One dose of MMR (measles, mumps, rubella) on or after the first birthday.
  • Second dose of measles (rubeola) or a second MMR administered 30 days after the first dose.
